The Chronic Healthcare Clinic provides a health home and continuity of care for patients with chronic medical conditions that require ongoing treatment, such as diabetes, high blood pressure, thyroid disease, and respiratory conditions.

Our part-time nurse practitioner serves as the Primary Care Provider, allowing our patients to be managed by the same provider during each visit. The continuity of care enhances the patient’s ability to follow the treatment goals, enables better self-management of his or her health status and can reduce complications, often resulting in the patient's ability to remain in the work force.

In addition to tracking patients’ status, the practitioner and nursing staff educate patients about their disease, treatment options, and the importance of compliance, all of which contribute to stabilizing the condition and delaying complications.

Chronic Healthcare Clinic visits are by appointment on Tuesdays and Thursdays.
